Equipment Surplus Procedures

Electronic & Computer Equipment

1. Download and fill out the "Equipment Surplus" Form (Fill out all columns except for Condition & Estimated Value)
2. Fax or email the form to Pepper Muae
3. Pepper will Contact ITI/ITO and let them know you have equipment that needs to be surplused
4. ITI/ITO will setup a time with you to pickup your equipment
5. When ITI/ITO picks up your equipment, give them a copy of the "Equipment Surplus" Form (Make sure the form is signed by your Department Head)
6. ITI/ITO will not pickup the equipment unless notified by Pepper first
7. ITI/ITO will then sale or dispose of the equipment and forward the "Equipment Surplus" form and/or "Equipment Disposal" form to Pepper with the CIO's signature when the surplus/disposal is complete
8. A week from the date of the surplus/disposal, your inventory list will be updated with the changes

For all other Equipment

1. Download and fill out the "Equipment Surplus" Form (Fill out all the columns including Condition and Estimated Value (if known) & The CIO Signature is not needed only the Department Head's signature)
2. Fax or email the form to Pepper Muae
3. Pepper will contact CDC to let them know you have items to surplus
4. CDC will then contact you to schedule a time for pick up
5. When CDC picks up your equipment give them a copy of the "Equipment Surplus" form
6. CDC will then sale or dispose of the equipment and send me the "Equipment Surplus" Form and/or "Equipment Disposal" Form when the surplus/disposal is complete
7. A week from the date of the surplus/disposal, your inventory list will be updated with the changes
