Mission Physicals

 

Preparing Ahead of Time

One of the first steps to completing your mission application is to have a complete physical. Your missionary application packet has two forms which you will need to bring with you to your appointment. These forms are the Personal Health History for Missionary Candidate” and the Physician’s Health Evaluation."

Cost

The courtesy cost of a mission physical is $35 for students, which includes the physical examination, urine analysis, hemoglobin blood test and vision eye screening. The cost of immunizations and blood draw fee for blood type and RH screening is not included. See below for individual vaccine costs.

Immunizations & Tests

In order to complete your mission physical in a timely manner, we suggest that you gather the listed information below before your appointment with the doctor.

* Immunizations received to date:

  1. Tetanus/Diphtheria/Pertussis
  2. Polio
  3. Hepatitis A
  4. Hepatitis B

* Blood type and Rh factor

You may also have the information faxed to the Health Center at (808) 293-3506. Please make sure the fax is address as follows.

Immunizations are available through the Health Center, and payment is required at the time of service. The cost of each vaccine/test is listed below (prices are subject to change).

Tests:

  • TB skin test (PPD) $20
  • Blood type and Rh factor $12

If You Have Had a Positive TB Test in the Past

If your TB test is positive and you have been exposed to tuberculosis, the Church Mission Department highly recommends that you start preventive TB medications before submitting your mission application. Preventive medications can be obtained free of charge through the Department of Health TB Control Program at Lanakila Health Center, 1700 Lanakila Avenue, Ground Floor, Honolulu, HI 96817 (ph 832-5731). For more information, contact a nurse at the BYU - Hawai’i Health Center at 293-3510.

Come With A Full Bladder!

Arriving to your appointment with a full bladder will help complete your physical exam more expeditiously. In other words, don’t go to the bathroom before you come!
